Voter Registration
Voter registration information is maintained on a State-wide Voter Registration System (SVRS). Voters must complete a registration form before voting for the first time in Wisconsin. They are also required to complete a new registration form whenever they move to a different address or if there is a name change.
Instructions
Please read the instructions that are with the registration form. Complete all areas of the form. Be sure to use the correct name as shown on the Wisconsin driver’s license or social security card.
When to register
In Wisconsin, a voter may register as late as Election Day. If registering by mail, the application must be postmarked no later than the third Wednesday before an election. New Wisconsin voters must provide an acceptable form of identification that includes their full name and current address.
Acceptable forms of identification must include current and valid photo identification (i.e. Wisconsin driver’s license or ID card), OR, one of the following: a copy of a current utility bill, bank statement, government check, paycheck, or government document. After the mailing registration deadline is passed, all voters must register in the Village Clerk/Treasurer’s office.
If registering in person, an applicant can register in the clerk’s office anytime during office hours until 5:00 p.m. on the day before the election. If registering at the polling place on Election Day, an applicant must bring acceptable identification with them.
Voter Information
Information is available at the Clerk/Treasurer’s Office, Government Accountability Board (GAB) elections website, and the Voter Public Access website.
Absentee Voting
If you are a qualified elector who has registered to vote, and you are unwilling or unable to appear at the polling place on Election Day, you may request an absentee ballot in writing, delivered to the Clerk/Treasurer’s office by letter, or in person. An original signature is required.
Requesting by mail
If you choose to request an absentee ballot by mail, your written request must include your full name, current village address, the address where the ballot should be mailed, and your signature. Your request should be mailed to the Clerk/Treasurer’s office, 138 E Main Street, Mount Horeb, WI 53572. Your request must be received no later than 5:00 p.m. on the Thursday immediately preceding the election.
In Person
If you choose to request an absentee ballot in person, you may do so at the Clerk/Treasurer’s office, 138 E Main Street. You may vote immediately or request the ballot be mailed to you. You cannot take a ballot home or pick one up for another voter.
By fax or email
The Clerk/Treasurer is permitted to send an absentee ballot by means of a facsimile transmission or electronic mail to an elector who files a valid application for an absentee ballot. The absentee elector must reliably inform the Clerk/Treasurer of a fax number or email address where the elector can receive an absentee ballot. Any faxed or emailed absentee ballot must be returned to the Clerk/Treasurer by U.S. mail. The ballot must be received by Election Day.
Permanent Absentee Status
If getting to the polls is a physical hardship, you can check the area on the application form for indefinitely confined status. A ballot will be sent to you automatically as long as you return each one.
